Sign up with a CPA network such as CJ or maxbounty, none of them require a phone call and they're both good.

Choose an e-mail submit offer or a single page offer that has a high epc, I like offers that target free ipods or mobiles, laptops, insurance estimates etc...

Let's say that you choose an e-mail submit offer that offers a free laptop for the winner.
E-mail submits usually pay $1-$4. It's not much but all you need to do to make $1-$4 is to get one person to submit his e-mail

Now just create an e-book that talks about laptops and in the bottom include something like "You can get a free laptop here" <---(That's the anchor text, put your link inside)

Now just distribute the e-book to as many people as you can. You can start off by torrents, forums etc...

The good thing about this is... the e-book can go viral!!

Imagine if just 100 people read your e-book and 20 of them enter their e-mail. you will earn $20 if the price is $1

Now you can do this as many times as you want

This method may sound "easy," but getting your ebook read, let alone getting it viral is the hard part. There are better ways to monetize ebooks with the same amount of viewers as in this method, imo.

As many as you want... the more you have the better

 

Just make your links in the ebook to point to a domain you own from where you can redirect to the offer. That way you can change the redirection if the offer changes or is canceled.

I like the idea of Kinky Joe, though often it takes some work before you start making this $ 20.

Different ad networks, different policies... Nowadays there are networks who immediatly call you (like an automated call) after sign-up to verify the affiliate account and provide you pin over the phone. Others it might take shortly before they will contact you. And there networks you never hear back from after filling out your information for whatever reason.

We often approve affiliate accounts on the spot or at least within 24 hours. If you have IM of their affiliate manager, don't wait but contact them... that really could save you time!
