For ppc and cpm, AdBrite would be your best choice for proxies. But for cpm, AdSense would be your best choice for blogs, not proxies.

Also you may try those cpm ad network Cpx Interactive, they might accept proxies, not sure thought.

You can start with AdSense for the homepage and AdBrite/Bidvertiser (better AdBrite) for the proxified pages.
Make sure you don't add AdSense on the proxified pages if you don't want to have your account terminated.

I wonder what you have done wrong, because AdBrite doesn't ban much members until they break their terms of service pretty bad.

Your last option would be AdToll or Cpx Interactive, thought not sure if they are going to accept your site.

Try Etology/AdEngage or contact me - I can offer you pretty decent CPM rates on personal traffic deals (1-by-1) provided you have a good amount of uniques/day.

For adbrite I was making 30$ a day with around 2000 uv a day and 30k pv on my forums and they banned me after 400$ was earned and they said your ad clicks are too high or what ever they said and they asked for a apache log so I send them it + awstats and they said sorry your account wont be unbanned.

Ill try adengage used them once. Etology sucks. Ill pm you

That sucks about AdBrite and loosing the $400 :(

I personally do not recommend using AdEngage, you should consider AdToll than AdEngage.
