which networks have incentivized offers?

which affiliate networks allow me to say on my site "complete this form/submit email, etc. and receive a free ...."

also, which affiliate networks have offers, where someone can order a free sample of a product and i get paid for it

and who pays the best for this and how much?

Incent? CPA Storm have a lot, Maxbounty quite a few, some on Azoogle...

 

My site is incentive and I use the ones below - all paying as well.

They all vary in offers and commission rates.

Promoting an email/zip submit offer that promises a free *whatever* does not necessarily mean it is incentivized. Incentivized means you are providing an additional incentive to your visitors for filling out that offer.
