You just need to add a good CAPTCHA system in place, and even the base version of PHPBB has ways to adjust that. There are also mods to make bot registration even less possible.

In my forum, I've done that and also check all incoming e-mails addresses and IP's for spam history at this site :

We've had two get through and post, in three months - and one was before we instituted the system.

I ended up blocking networks and domains and aol... it got so much hassle that now every post is manually read before its going live.

The new spam bots have broken the captcha system. Even with email activation, the bots still sign up and post.

There are a couple of things you can do to stop the spammers:

Install ReCaptcha and use it instead of regular captcha.

Install a question and answer modification. Or if your forum has this built in, turn it on and use it along with ReCaptcha. A question and answer modification means that the person will have to answer a question before they can register. An example of this is - "What is 2 + 2?" and the person has to enter 4 or four.

Image verification - these modifications have you upload several images, and the person has to click a certain image. Lets say that there are 3 pictures of a dog, and 1 picture of a cat. Then you set the question so the person has to click the picture of the cat. But even with a bot, there is a 25% chance they will pick the right image. And, these are language dependent. Meaning if someone has limited knowledge of your language, they might not understand the question.

Add a few more required questions to the register form. Such as are you male, female, rather not say.

Any one of those will slow down spam bots, and a combination of any two will stop the spam bots. But the human spammers will still get in. On my main forum I use recaptcha and one more required question. And I rarely ever get spammed. My wifes and a my sons forum were getting hit by spammers pretty bad until I installed a ReCaptcha modification. Just by using ReCaptcha, that stopped 99 - 100 percent of the spam bots.

    use email activation

means a user cannot be registered until he gets an email having activation link.

so that you can stop spammers from registering.

you can disable any signatures for sometimes just as dp. users have to post min 50 post to get signature.
    you can also have mods.

This does not prevent spammers.. I use this procedure and spammers are still doing their work.

There isnt much you can do to stop "real" people posting spam, but this post shows some ways to stop the bots and autospammers: Block Bots and Spammers
