$50 Thank You Note Design [Paypal]

Hi,

I just want to say "Thank you".
I would like you to design a two sided note which has the look and feel of my website http://tinyurl.com/5zw48y

Characteristics:
- 2 sided
- 210mm width and 99mm hight
- Page layout: landscape
- white margin below, under and sides of 15 mm
- Will be used for printing: minimum of 300dpi and CMYK colours (important!)
- final result should be editable photoshop file
- look and feel of website http://tinyurl.com/5zw48y
- Use a mainly white background and not totally blue for example
- Side 1:
- Use logo ( http://tinyurl.com/a7pw59 )
- Include logo slogan: Iedereen verdient de waarheid
- Text: Hartelijk dank voor uw bestelling (=thank you in dutch)
- Side 2:
- here should be place to add some lines of text. "Questions? blabla..
- an idea might use a layout like a normal page with footer, header, sidebar
- reward: $50 by Paypal and my neverending appreciation

I hope I explained well?

I'll give feedback for every entry and I'll check the results at least once a day, probably more.

I already had some wonderful results during previous contests and I'm sure this will be a success as well.
