Hey I have a question about how the Content Network picks sites to show your ads on. Let's say you have group A with 100 keywords that show up on X sites. Let's say you have another group B with another 100 different keywords that show up on Y sites. If you make 1 group with keywords from A and keywords from B combined, will the sites you show up on be (X + Y) ?
Make 50 small (10-20 keywords) ad groups and run a placement report after 2-3 days. You'll see which sites and ad groups convert for you to make new ad groups based on statistic

Yes, it is best, although time consuming, to group keywords pretty tightly so that you can see what is working best. You'll save a lot of money if you test and track. If you have little money and lot of time - do a lot of testing and tracking. If you have little time and lot of money - throw a bunch of keywords out there and see what sticks.
