Need a PHP recommend script

Was wondering if someone can point me to a simple script to have a form on a web page to recommend site to friends. Nothing fancy just a form for user to input their name and email address of friends to send an email to recommend the web site.

I plan on putting this in a small section on the main page so ideally it would be great that when the user clicks "submit" the section changes from the form to a notification saying "You recommendation has been sent"

If this is impossible (not sure if it doable or not) I could also just create an exact clone of the main page and have the redirect point to that.

Would appreciate any help. All of the ones I found usually have a pop up box for the form and I would really like it to operate on the main page.
