how to create a floating subscription form

Hi all

I want to create a floating subscription form to my website.
can some1 pls. help me how can in how to do it, any tutorial website or something.

I beleive it is done using javascript or ajax.

what I mean by floating subscription form?
a form that will move while you are scrolling, and also has a close button so user can close the form

 

what happens if I don't have javascript turned on because I'm afraid of auto-running scripts like this when I go to your website?

 

so is there any other way to without javascript or ajax?

if so could you pls. share it how
