You can put all the details in the Paypal notes when you make the payment.
You can pay the $7.50 per site by clicking here.

First come, first served. All work should be carried out between 24-48 hours from time of payment (depending on how many jobs I get).

It's with a script. Manually submitting takes a lot of time and I would be charging a lot more than $7.50.

PR ranges from PR0 to PR8. I have no idea how to find out which are what PR.
Approval depends on your site. As long as it is not objectionable content then should be around the 94% mark (this takes time though).
