Here is a free (and SUPER) easy tool to put a survey on your website. No kidding, this takes less than a minute. A cool thing about it is that you are free to reformat the HTML however you want to match your site design.

Yes, there are many benefits of coding applications like this, yourself. Then you have complete control over the functionality, the display and the data collected

Well, since all it does is throw PHP errors, I can't say I'm convinced that it's "better."

You got on error my site (sfp.cc) or theirs? Don't use their demo, they forgot to permission the files correctly. The setup is easy enough though (<15 minutes)
