W-8BEN Form - Need Help

    I just got accepted by azoogle but they ask me to mail them this form:
    www[dot]irs.gov/pub/irs-pdf/fw8ben.pdf


    what i have to write at 10? (i'm from Canada)

    And what Statement exactly i have to provide? (11)
