    1. Clicking your own ads
    2. Getting someone else to click your ads
    3. Putting prohibited content (especially porn) on your site

    i did not banned from adsense but i think most banned reasons are

    clicking on your own ads
    illegal content (adult , mp3 etc)
    using automatic traffic programs
