It is a free module for registered users, spamming is common in any forums ( Russian hackers :) ) try this module. Also lot of other options also there.

How you are allowing users to register ? By clicking the mail link ?

What version of vB are you using? Try to upgrade at the latest and turn on all or at least one anti-spam feature, one important thing is to add some security questions.

    See this thread: http://www.vbulletin.com/forum/showthread.php?t=275800

Here's what I do to prevent bots from spamming:
I use the native reCaptcha Human Verification method in vBulletin. I also have installed the modification, "NoSpam!" from vbulletin.org.
This is essentially using two Human Verification methods, since vBulletin only allows you to use one at a time.
It has worked very well in terms of stopping bots from registering.

To prevent spam from appearing on my board, I have set up a promotion where the first two posts of a new member must be approved by a moderator. This way, I don't have to keep my eye out for spam, and users don't have to see it or report it.
I have a notice displayed to new members informing them of my 2-post-moderation procedure, so I'm sure that will turn away some human spammers (they know they can't get past it).

Use NoSpam mod from vbulletin.org, with just one question.
    http://www.vbulletin.org/forum/showthread.php?t=183329
This is for VB 3.7, but works for 3.8.1 as well.

Simple rules to frame the question:
    1) The answer for the question should be unique.
    2) Avoid questions a bot can answer correctly by presenting a part of the question as answer. ( eg: Which is grerater, 7 or 8? )
    3) Dont use mathematics ( 2 + 3 = ? )
    4) Avoid Yes or No type questions. (Eg. Are you a boy?)
    5) Change the questions once in a week, or when you realise that a bot has crossed the NoSpam question.
    6) Dont use questions which will have one letter answer. (Eg. Which is the first letter of English alphabets?)
    7) Think of all the possible correct answers a human might give for the given question. (Eg delhi:newdelhi:new delhi etc.)
    8) Remember that the answers are not case sensitive.

Examples of good questions in my opinion:
What is the color of blood?
What is the color of leaf?
What is the capital of India? (I am using this question for the past 2 weeks. No bot is able to get through.)

BTW, I no more use capcha or recapcha.
They are in my opinion additional redundant burden to prospective members. :)

Try to use questions related to your niche. For example, if your forum was about birds, you could ask questions like "how many wings does a bird have?" or "what do birds eat: seeds or pizza?"
