Form to Mail script, with blacklist option

Hi all, i'm not new here but i haven't posted so far, tho i have read many good threads so far :)

i need help with form to mail script, i have spent whole day today searching for solution, and have checked DP search also but haven't found answer :(

What i need is script that will send info to my email (this is simple) but what i couldn't find anywhere is that kind of script with blacklisting options... i need to be able to blacklist people by domain name (website.com), by email adress (*@domain.com) and IP adress... can anyone help me please? :)
