$25 Animated Banner Contest

I am looking for a banner that I will use for advertsing purposes. I am currently running a car insurance website (http://www.quotearena.com/AutoLanding.html). If you take a look at it you will see that there is a spot that gives the user the ability to type in a ZIP code. Once the user clicks on the button a JavaScript presents the local insurance providers. What I want is a banner that has the user input his/her zip code into it, and he/she is directed to the results page. The banner should look professional and not generic. One possibility it to have a picture in the background, such as the one seen here (http://www.istockphoto.com/file_closeup/business/business_concepts/insurance/2860278_insurance_policy.php?id=2860278).

The size should be 468x60.

Hey nice look! ... i apologize I didnt specify on the text .. it should say "Save Hundreds on Car Insurance" and " Enter Your ZIP Code Here" .. could you make it so I can test the script? .. thanks

Not to fond of the banner Krates... the text box should be smaller and only allow 5 characters... also QuoteArena should be displayed on the banner .. possibly put some images of cash ..
