$0 balance "Plain" statements intermingled with "Fancy" Statements

We started printing off our customer statements today.
We noticed several statements printed out in "plain" format, not our normal format. The customers who received this type of statement do have a balance, but the the "plain" statement showed a $0 balance. There does not seem to be a pattern we can find.

Is there somewhere we can look?

Any possibility you could scan/fax a copy of each type you are seeing?

Probably not unless you have a log keeping script.
